I certify that to the best of my knowledge, the information that I have given in my application for employment is true and complete and understand that any false statement or omission to Shield Security Services or its representatives I will be liable to disciplinary procedures that could lead to termination of employment without notice. I understand that it is a criminal offence to make false statements on this Application Form.
I understand and agree that if so required I will make a Statutory Declaration in accordance with the provisions of the Statutory Declarations Act 1835 in confirmation of previous employment or unemployment.
I authorize Shield Security Services or its agents to approach Government agencies, former employers, educational establishments, criminal justice agencies and personal referees for information relating to and verification of my employment/unemployment record. I consent to Shield's reasonable processing of any personal information obtained for the purposes of establishing my medical condition and future fitness to perform my duties. I accept that I may be required to undergo a medical examination where requested by Shield. Subject to the Access to Medical Reports Act 1988.
I consent to the results of such examinations to be given to Shield Security Services and authorize Shield to make a consumer information search with a credit reference agency, which will keep a record of that search and may share that information with other credit reference agencies.

Working Time Regulations 1998
The Working Time Regulations govern the hours most workers can work and sets a limit of an average 48 hours a week on the hours a worker can be required to work. Individuals may choose to work longer by "opting out".
The Directive states that the security industry is not bound to comply with regulations relating to night workers working longer than eight hours in twenty four, rest periods of eleven hours per day or one day per week or a rest period every six hours worked, provided you are allowed the same rest time at a later date.
Under these regulations Shield Security Services must obtain your written permission if you wish to work more than 48 hours per week. The agreement to opt out is optional. The Company will not expect you to work more than 48 hours per week unless you indicate you wish to. Furthermore your 'opt out' can be withdrawn at a later date.

- The Working Time Regulations 1998 provide that the average working time including overtime should not exceed 48 hours for each 7 day period. The Company and the Employee agree that this limit shall not apply to the Employee
- This Agreement will remain in force until such time as the Employee or the Company gives not less than one month's notice in writing to terminate it.
