1. Personal Details

Please note: we only ask for your date of birth in order to meet the requirements of BS 7858:2019, the standard for security screening. The recruitment team will not be able to see your date of birth during the decision-making process, and it will have no bearing on whether you are employed.

2. Address History (5 Years)

Please provide the last 5 years of address history starting with your current address. Must include postcode if within the UK.

3. Right to work

Please evidence Right to Work in the UK by providing the relevant number from one of the documents below. A UK passport is the most straightforward. You will need to be bring the original to your interview. List A documents are permanent; List B documents will be checked again during employment.

13. Working Time Directive Opt-Out

The Working Time Directive means we can’t require you to work more than 48 hours a week on average. If you want to work more than 48 hours a week on average, we need to have your written permission which you can give by signing the form below. If you’re happy to work 48 hours or less each week, then you don’t need to sign this.
I agree that I may work for more than an average of 48 hours a week. If I change my mind, I will give my employer 1 months’ notice in writing to end this agreement.

14. Confidentiality Agreement

During your employment with Shield Security Services (Yorkshire) Ltd, it is likely you will gain access to information, electronic workstations or see images of a sensitive, private or confidential material either of a physical nature (site operations, personnel, keys, etc.), or of an intellectual nature (passwords, PIN numbers, practices, procedures, etc.).

It is an express requirement of this agreement that you do not at any time abuse any access to such material, or disclose any information, or discuss any Client’s business with anyone else other than your immediate manager who will then reserve the right to inform the Client at his/her discretion.

You should report any potential breach of confidentiality; this could include:

- Inappropriate disclosure of information to a third party or any external source
- Inappropriate use of social media

In addition:
- Client identifiable information must not be shared with any third party
- No person identifiable information is to be communicated to any external organisation, person or third party

This agreement applies for the full term of your association with our company and an indefinite period afterwards.

Any such breach of this agreement may render you liable to disciplinary actions that may result in suspension and/or dismissal. The company also reserves the option of prosecution and to make such appropriate deductions from wages in respect of losses incurred by either client, members of the public or Shield Security Services (Yorkshire) Ltd., and/or its workforce.

15. Uniform

Shield Security will supply the uniform necessary for your role on a loan basis. This will require you to make a refundable uniform deposit payment of £100. This will be deducted from your wage slip at £25 per month for 4 months.

Should your employment with us end, you must return all items of loaned uniform in a good, clean condition, otherwise the deposit may not be refunded.

By signing below, you confirm your agreement to the above, and understand that it will form part of your terms and conditions of employment with Shield Security Services (Yorkshire) Ltd. You understand that you are required to return the items in good, clean condition at termination of employment, at which point your deposit will be refunded. You understand that if you fail to do this, you will not receive any refund.

16. Declarations

I understand that employment with this company is subject to references and screening in accordance with BS 7858.

I confirm that the information I have provided on my application is true and complete to the best of my knowledge. I understand and agree that I will be subject to any or all of the following checks:

- Address check
- Financial probity check which the company will retain on file
- ID verification checks
- Academic/professional qualification check
- Employment history, including any periods of unemployment/self-employment and any gaps
- Criminal background check

I authorise the company or its agents to approach government agencies, former employers, educational establishments, for information relating to and verification of my employment or unemployment history, a consumer information search and ID check with a credit reference agency, which will keep a record of these searches in line with current legislation.

I further declare that any documents that I provide as proof of my identity, proof of address, and any other documents that I provide are genuine and that any falsified documents may be reported to the appropriate authority. I understand that it may be a criminal offence to attempt to obtain employment by deception and that any misrepresentation, omission of a material fact or deception will be cause for immediate withdrawal of any offer of employment.

I accept that I may be required to undergo a medical examination where requested by the company.

I understand and agree that if so required I will make a statutory declaration in accordance with the provisions of the Statutory Declarations Act 1835, in confirmation of previous employment or unemployment.

I understand that any false statement or omission to the company or its representatives may render me liable to dismissal without notice.

By signing this declaration, I agree that I have provided complete and true information in support of the application and that I understand that knowingly making a false statement for this purpose is a criminal offence.
